Your Guide to Residential Aged Care
Do you need daily support, short-term respite or permanent Care?
Before you enter residential care you need to be assessed by an Aged Care Assessment Team (ACAT).

- Once this assessment has been completed you will receive a copy of the Aged Care Client Record (ACCR) which will advise as to the level of care required.
You must understand the costs involved. While the Government subsidises aged care, how much you pay depends on your financial situation.
- This is determined by Centrelink once you have completed and submitted the Request for a Combined Assets and Income Assessment Form (SA457). Alternatively, you can download this form here.

On admission, the resident will be required to bring his/her:
- Medicare Card;
- Pension Card (if applicable);
- Pharmaceutical Card and Safety Net number (if applicable);
- Private Health Care Card (if applicable);
- Current medications and prescriptions;
- Assistive devices (walking stick/frame, wheelchair etc.); and
- Personal Effects.
